Insurance, Payment, and Appointment Policies

Insurance Information

Precision Physical Therapy is in-network with the following insurance plans:

  • Kaiser

  • Regence

  • Uniform Medical Plan

  • Blue Cross Blue Shield out-of-state and federal plans

  • Teamsters

  • Anthem

  • Aetna

  • Moda Health

  • First Choice Health

  • HMA

  • Cigna

  • L&I, including Washington State and private claims

  • TriWest

  • Tricare

  • Auto insurance

Premera BCBS

Precision Physical Therapy is out-of-network with Premera BCBS.

If you have Premera BCBS, we can run a benefits check to help estimate your out-of-network coverage. Many patients with Premera plans have similar in-network and out-of-network benefits, but coverage varies by plan.

Self-Pay Rates

Self-pay rates are available for patients who choose not to use insurance or who have high-deductible plans.

We can also provide a superbill that you may submit to your insurance company to apply toward your deductible.

Payment Expectations

Payment is due at the time of service for copays and self-pay visits.

We accept:

  • Cash

  • Check

  • Credit and debit cards

  • FSA and HSA cards

Benefit Verification

As a courtesy, Precision Physical Therapy provides a benefits check for each patient. This is an estimate of covered services and is not a guarantee of payment or coverage.

Patients are responsible for verifying their own benefits, as coverage details may change at the time of service. For exact benefit information, please call the member services phone number on the back of your insurance card.

Medicare

Precision Physical Therapy is not currently contracted with Medicare.

At this time, we are unable to bill Medicare directly or accept cash payment from Medicare beneficiaries for physical therapy services.

Cancellation and No-Show Policy

Your scheduled appointment time is reserved specifically for you.

If you need to cancel or reschedule, please call, email, or text your physical therapist at least 24 hours in advance. This allows us to offer the appointment time to another patient.

Appointments canceled with less than 24 hours’ notice, or missed appointments without notice, will be billed a $60 late cancellation or no-show fee.

If two late cancellations or no-shows occur, Precision Physical Therapy reserves the right not to schedule additional appointments.
