Insurance Information

In-Network

*Patients are responsible for contacting their insurance company to understand in-network benefits including deductible, coinsurance, and copays prior to your PT evaluation.

*We must bill insurance for Physical Therapy services, if we are contracted/ in-network with your insurance. Cash pay is only available for clients who are uninsured, or those with Out-Of-Network coverage.

Self pay of $150 per session are available for patients with out-of-network insurance plans.

*A superbill can be provided for you to submit to your insurance for potential direct reimbursement.

Medicare

Precision Physical Therapy is not contracted with Medicare. Due to laws surrounding Medicare, we are unable to accept cash payment from clients for physical therapy services, and clients must seek PT services from an in-network clinic.

Payment Expectations

Payment is due at the time of service for copays and self-pay visits.

We accept:

  • Check

  • Credit and debit cards

  • FSA and HSA cards

Cancellation and No-Show Policy

Your scheduled appointment time is reserved specifically for you.

If you need to cancel or reschedule, please call, email, or text your physical therapist at least 24 hours in advance. This allows us to offer the appointment time to another patient.

Appointments canceled with less than 24 hours’ notice, or missed appointments without notice, will be billed a $60 late cancellation or no-show fee.

If two late cancellations or no-shows occur, Precision Physical Therapy reserves the right not to schedule additional appointments.
